ANEW Place is seeking a compassionate, organized team player and problem solver to provide case management services to adults experiencing homelessness in the Greater Burlington area and residing in or graduating from our congregate transitional shelter facility. ANEW Place is a faith-based homeless services agency equipping adults in the Greater Burlington area experiencing homelessness with the tools for lifelong change.

THE OPPORTUNITY: The Case Manager reports directly to the Program Manager. Primary responsibilities include assisting in managing a waitlist, screening applicants and conducting intakes for entry into the four-phase program offered at ANEW Place, reaching out to guests in a culturally competent manner and working with guests to develop individual case management plans, monitoring progress toward short and long term guest goals, offering assistance to guests in accessing available services, providing accountability, support and advocacy for guests, collaborating in program planning and the assisting in the overall operation of the shelter.

CANDIDATE CRITERIA:

· Compassion for low-income individuals and understanding of issues pertaining to those affected by poverty and homelessness

· Bachelor’s degree in social work or a related field, and/or experience in providing case management services in a shelter setting

· Familiarity with homeless, mental health, substance abuse and housing resources in the Greater Burlington area preferred.

· Ability to maintain case management records, prepare written documents, reports and other materials

· Ability to use internal and external electronic tracking systems and database software

· Strong communication and organization skills

· Ability to work autonomously at times
